No designer credited. The photograph is by Friedman-Abeles and was also used on the poster for the stage production at Detroit’s Fisher Theatre.

The title on the cover wasn’t just reproduced from the setting for the poster. A comparison shows that the words were reset, with a bit more slant, different spacing, top-heavy e’s, and also different glyphs for the final three letters (Caruso Roxy had seven forms for i).
