808s & Heartbreak is the fourth studio album by rapper Kanye West. From Genius:

The album is heavily influenced by 1980s synthpop and electropop. Kanye used the “808” sound and altered its pitch to create the unique distorted and electronic sound you hear throughout the album. He called this effect the “heartbreak,” and said that it was ‘representative of his state of mind’.

According to Discogs, the package design is by JP Robinson, with art direction by Past Tell Museum, and art and photography coordination by Kristen Yiengst. An article on Complex credits the photo of the deflated heart-shaped balloon to Kristen Yiengst and and the art direction to Virgil Abloh and Willo Perron, working under the Past Tell Museum name (the creative group precursor to DONDA). New York-based artist KAWS created the deluxe album cover (not shown here).
